Please try to come at least 15 to 30 minutes prior to  your appointment.

This will allow our staff to scan your paperwork and enter biographic information prior to your visit.  

Welcome New Patients

Is this your first time visiting us?   It is important to all of us at Calvary Medical Clinic to know and understand our patient’s wants and needs. We make sure we have enough time set aside for all of our patients to ensure you have a thorough visit with us. Therefore, we ask for you to arrive thirty (30) minutes prior to your scheduled appointment time on your first visit if you have not already downloaded and filled out paperwork from our web site.   

Also, if you have medical insurance, please bring your card with you and identification with you.   If you have already filled out your paperwork, you may arrive fifteen (15)  minutes early for your scheduled appointment
